We currently accept assignment on all Medicare and Medicare Advantage Plans. We participate with most managed care plans in Northeast Indiana. If we are not contracted with your plan, you will be responsible for paying for your office visits and treatments at the time of service. As a courtesy, we will file your claim for you. A list of the major insurance companies we participate with is listed below, but please contact your insurance company to confirm that we are still participating with them.
